Founded in 2017 with a focus on bringing modern healthcare to local communities in need, Cityblock provides primary care services, behavioral care, chronic disease management, substance abuse as well as social services assisting with transportation, housing and healthy living.

CMG began its partnership with the healthcare provider to assist in its expansion of Neighborhood Hubs (or clinics). Managing the coordination beginning with site selection through construction, CMG continues to assist the team through its development of clinics in North Carolina, Connecticut and New York.
